This form tells us information necessary to establish an Address Book in our Messaging Center providing access for creating messages to be transmitted on the DataFM network. Each person who will be be creating messages for broadcast on the network should have an Address Book - we do not recomment sharing your Address Book. A form should be completed for each people needing an Address Book.

TAXABLE Enter "N" if you are a government agency or tax-exempt

PAYMENT FREQUENCY

AUTH PERSON NAME Enter the name of the owner of this Address Book

AUTH PERSON TELEPHONE Enter a telephone number for the owner of the Address Book

AUTH PERSON EMAIL Enter the email address of the owner of the Address Book

There are two ways you can complete this form:
